Shoreditch Park and City Primary Care Network (PCN) is the collective name for our local practices who have come together to focus on local patient care. The practices within the network are Shoreditch Park Surgery, De Beauvoir Surgery, Neaman Practice, Southgate road/Whiston Road Surgeries, Hoxton Surgery and Lawson Practice. By working closely together, we are strengthening the health services around our local population. We want to hear your views and work with you to build health services that benefits everyone in our communities.
By taking part in this survey, you will help us understand what is working well and what isn’t, where you’d like us to improve and focus and what services you would like to see develop in the future in your local area. We’re really interested in hearing your ideas and there are more opportunities to be involved in more in depth discussions, all the details are at the end of the survey. Equally, you can be anonymous as well, we don’t mind!
