Navigation regularly involves connecting the resident to a range of support and services, both community-based and statutory.’

Multidisciplinary Meetings (MDMs) bring together everyone involved in someone's life to ensure they’re up to date and work together in a more coordinated way to address all their needs.’
‘Neighbourhoods aim to bring health and care services closer to where people live, and help people take more control over the factors that affect their health and wellbeing. Through regular Neighbourhood Forum meetings and action groups in the community, this means that local services and plans will be informed and shaped by the experiences and priorities of local people.’
